29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: 176th St. & Patrick Ave., Omaha, NE 68116 (Job-site): Employees were using a supported scaffold platform that was being used with a powered industrial truck. The scaffold was not secured to the forks.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented: 176th St. & Patrick Ave., Omaha, NE 68116(Job-Site): The employer did not ensure that adequate fall protection was provided for employees who were working on the roof of a home which was approximately 18 feet from a lower level. Affordable Exteriors Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA Inspection number 314059387, Citation Number 2, Item Number 1 and was affirmed as a final order on March 25, 2011, with respect to a workplace located at 1206 South 199th Street, Omaha, NE 68130. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed.
