29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     The employer is failing to protect employees from fall hazards when performing framing activities.  This was most recently documented on the job site at 3909 N. 191st Street, Elkhorn, NE 68022 where employees were observed installing roof sheathing on a new residential home without the use of fall protection.  Instances include but are not limited to:  i.   Installing sheathing on the southwest corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.   ii.   Installing sheathing on the front side of the home for 30 minutes without having the catch platform in place or using  any other form of conventional fall protection.  iii.  Installing sheathing on the southeast corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.   iv.  Installing sheathing on the northeast corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.   v.   Installing sheathing on the northwest corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.     vi.   Installing sheathing for the roof along the south side of the home while the catch platform was removed to bring additional sheathing to the roof at least once or as much as three times lasting from 3-4 minutes each time leaving the employee exposed for up to 12 minutes without having the catch platform in place or using any other form of conventional fall protection.    vii.   Installing sheathing for the roof along the north side of the home while the catch platform was removed to bring additional sheathing to the roof at least once or as much as three times lasting from 3-4 minutes each time leaving the employee exposed for up to 12 minutes without having the catch platform in place or using any other form of conventional fall protection.    viii.  Installing sheathing for the roof along the east side of the home while the catch platform was removed to bring additional sheathing to the roof at least once or as much as three times lasting from 3-4 minutes each time leaving the employee exposed for up to 12 minutes without having the catch platform in place or using any other form of conventional fall protection.    ix.  Installing sheathing for the roof along the west side of the home while the catch platform was removed to bring additional sheathing to the roof at least once or as much as three times lasting from 3-4 minutes each time leaving the employee exposed for up to 12 minutes without having the catch platform in place or using any other form of conventional fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer is failing to provide adequate training to employees on the recognition of potential fall hazards and on methods or procedures that may be used to mitigate the risk of falling such as the implementation of various forms of fall protection/restraint systems while working on residential homes where the ground to eave height is greater than 6 feet.  The most recent example of this was found on the job site located at 3909 N. 191st Street, Elkhorn, NE 68022 where the employer has untrained employees installing the roof sheathing on a new residential home without the use of fall protection.  Instances include but are not limited to:  i.   Installing sheathing on the southwest corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.   ii.   Installing sheathing on the front side of the home for 30 minutes without having the catch platform in place or using  any other form of conventional fall protection.  iii.  Installing sheathing on the southeast corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.   iv.  Installing sheathing on the northeast corner of the home for 15-20 minutes without having the catch platform in place or using any other form of conventional fall protection.   v.
