29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): On or about May 22, 2013, on the rig floor, there were three instances where the guardrail system did not provide fall protection. Employees were exposed to fall hazards up to 5-feet 2-inches to the ground. a) To the rear and right of the rig, next to where the operator stood there was a gap of 20-inches between the rails, the fall distance was approximately 4-feet 6-inches to the ground. b) To the rear and right of the rig floor, there was a gap of 24-inches between the rails; the fall distance was approximately 5-feet to the ground. c) To the rear and the left of the rig floor, there was a gap of 32-inches between the rail and structure beam, the fall distance was approximately 5-feet 2-inches to the ground.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: On or about May 22, 2013, the stairways attached to and leading up to the rig catwalk did not have railing on both sides and/or not secure. Employees were exposed to fall approximately 4-feet 6-inches from top of the stairs to the ground. a) In front and to the right of the rig, the handrail to the stairway was not attached properly and was not it secure. b) To the rear and right side of the rig, the stairway leading up to the operator's station did not have handrails on the right side.
