29 CFR 1926.351(b)(2): Cable free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder is connected was not being used: On or about June 4, 2013, an employee on the 2nd floor of the phase 3 construction section was exposed to an electrical hazard from an AC/DC welder supplying power through a welding cable that had unprotected wires; within 1 inch of the electrode holder.
