29 CFR 1926.760(a)(1): 29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet* (4.6m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: On or about September 23, 2016, four employees were installing metal decking on the third floor of the commercial property under construction and were not utilizing a complete fall protection system, exposing employees to a fall hazard of approximately 30 feet.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about September 23, 2016, the employer failed to notify the Occupational Safety and Health Administration (OSHA) of an injury that required inpatient treatment and an overnight stay at the hospital. OSHA became aware of the injury on September 25, 2016.
