29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions. On or about July 21, 2021, and at times prior thereto, employees were performing leading edge operations while using a Self-Retracting Device (SRD) that was not designed for the activity, exposing the employees to the hazard of falling to a lower level from heights of approximately 26 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. On or about July 21, 2021, and at times prior thereto, employees working on a leading edge were not using Self-Retracting Devices (SRDs) per the method required by the equipment manufacturer, exposing the employees to the hazard of free falling more than 6 feet or more to a lower level.
