Location: 2216 Avenue Brooklyn, NY.   On or about: 06/06/13   a) Two employees were installing ceiling joist from a tubular welded frame scaffold that was not provided with any fall protection system.  The employees were exposed to a potential fall hazard of up to 20 feet to the ground below.  b) Employees were working from an approximately 20 feet high scaffold platform that was not provided with fall protection.

Two employees were working by the edge of a woking platform without any fall protection while exposed to a potential fall of approximately 20 feet to ground below.
