Maintenance employees were utilizing locks that were keyed alike for the control of hazardous energy.

The openings in the electrical boxes for the conductors were not closed. The edges around the openings provided abrasion hazards to the conductors.

An emergency eyewash station in the vicinity of the aluminum anodizing area was not in compliance with ANSI 358.1-1998 for Emergency Eyewash and Shower Equipment in that the eyewash station was not activated weekly to verify proper operations. Weekly inspections of the equipment were not conducted.
