Each lockout or tagout device shall be removed from each energy isolating device by the employee who applied the device:       a) American Tinning & Galvanizing Company / 552 West 12th Street, Erie, PA: On or about June 7, 2013, lockout devices (i.e. locks) were capable of being removed under normal conditions by personnel who did not apply the lockout device. Maintenance employees were utilizing locks that were keyed alike for the control of hazardous energy.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm:    a) American Tinning & Galvanizing Company / 552 West 12th Street, Erie, PA: On or about June 11, 2013 in the aluminum anodizing area in the South West corner of dip room 2 the control for the 500 pound hoist had exposed wires. The outer jacket of the cable going into the electrical box came down and exposed the wires underneath.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) American Tinning & Galvanizing Company / 552 West 12th Street, Erie, PA: On or about June 7, 2013 company employees were working around various chemicals used in the metal plating process. An emergency eyewash station in the vicinity of the aluminum anodizing area was not in compliance with ANSI 358.1-1998 for Emergency Eyewash and Shower Equipment in that the eyewash station was not activated weekly to verify proper operations. Weekly inspections of the equipment were not conducted.
