29 CFR  1910.147(c)(7)(i)(B):Affected employees were not instructed in the purpose and use of the energy control procedure:  a.) American Tinning and Galvanizing Company; 552 West 12th Street, Erie, PA - On or about 8/22/23, and at times prior thereto, affected employees were not receiving lockout/tagout training. Employees worked with and around cranes, conveyors, and forklifts.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  a.) American Tinning and Galvanizing Company, 552 W. 12th Street, Erie, PA: On or about 8/22/2023 and times prior thereto, a Mitsubishi forklift (model: FGC25N) (serial: AF82F52925) was being used with a man basket to lift employees.  The company had not received the forklift manufacturer's approval to use the attachment.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

American Tinning and Galvanizing Company, 552 W. 12th Street, Erie, PA: On or about 8/22/2023 and times prior thereto, a Mitsubishi forklift (model: FGC25N) (serial: AF82F52925) was being used with a man basket to lift employees.  The forklift plate had not been changed to show the attachment. 29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by  moving parts, point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a.) American Tinning and Galvanizing Company, 552 W. 12th Street, Erie, PA: On or about 8/22/2023 and times prior thereto, a Mitsubishi forklift (model: FGC25N) (serial: AF82F52925) was being used with a man basket to lift employees.  The employees were exposed to moving chains and the mast of the forklift while being lifted in the man basket.  The basket was not equipped with shields, barriers, and/or guarding to keep employees hands and other body parts out of the moving parts associated with the forklifts mast.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a.)  American Tinning and Galvanizing Company; 552 West 12th Street, Erie, PA - On or about 8/22/23, and at times prior thereto,  employees were not receiving specific Hazard Communication training upon initial hire when working with and around chemicals.  Chemicals the employees work with and around, included, but were not limited to, sulfuric acid,  galvanizing flux, and  chromic acid during dipping/anodizing of aluminum onsite.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). American Tinning and Galvanizing Company -  On or about 8/22/23, and at times prior thereto, the company was not filling out Column F of the 300 log in the detail required.  The company did not record the injury on the log.  This occurred on recordable #7 in Calendar Year 2023.
