29 CFR 1910.29(a)(3)(v): The work level platform of scaffolds (towers) shall be of wood, aluminum, or plywood planking, steel or expanded metal, for the full width of the scaffold, except for necessary openings. Work platforms shall be secured in place. All planking shall be 2-inch (nominal) scaffold grade minimum 1,500 f. (stress grade) construction grade lumber or equivalent. a. On or about June 14, 2013, in the facility, employees were required to work on manually propelled mobile scaffold(s). The employees were exposed to fall hazards of approximately ten (10) feet when the scaffold work platform(s) were not planked the full width of the scaffold.

29 CFR 1910.29(a)(3)(vii): All work levels 10 feet or higher above the ground or floor shall have a guardrail of 2- by 4-inch nominal or the equivalent installed no less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1- by 4-inch nominal lumber or equivalent. a. On or about June 14, 2013, in the facility, employees were required to work on manually propelled mobile scaffolds. The employees were exposed to fall hazards of approximately ten (10) feet when the scaffold(s) did not have proper guardrails and/or mid-rails on each side of the scaffold work platform.

29 CFR 1910.177(c)(1): The employer shall provide a program to train all employees who service rim wheels in the hazards involved in servicing those rim wheels and the safety procedures to be followed. a. On or about March 20, 2013, an employee was injured when servicing a multi rim tire. The employer failed to have developed or provided an employee training program to ensure employees followed safe procedures when servicing single and/or multi piece rim wheels.
