Employees exposed to the hazards created by welding, cutting, or brazing operations shall be protected by personal protective equipment in accordance with the requirements of 1910.132. Appropriate protective clothing required for any welding operation will vary with the size, nature and location of the work to be performed. a. Repair Shop Building - On or about February 9, 2015, the employer had not provided employees with adequate protective clothing from the hazards of torching, cutting, welding operations. Employees were thereby exposed to fire hazards .

Repair Shop Building - On or about February 9, 2015, the employer had not selected a safe design of personal protective equipment (PPE) for employees conducting torching, welding and cutting operations. The following are examples of unsafe designs of personal protective equipment provided to employees at the shop: 1. Radnor - leather cape sleeves available to mechanics at the repair shop. 2. Tingley - Safety Vest - 100 % Polyester available to mechanics at the repair shop.

Yard's administrative Office/Mobile Trailer/Unisex washing facilities - On or about March 4, 2015, the employer provided washing facilities to employees without running water. Employees were thereby exposed to unhealthy and unsanitary working conditions. The locations without running water in the washing facilities were the following: 1. There was no running water in the lavatory to wash hands. 2. There was no water in the toilet's closet to flush.
