29 CFR 1926.761(e): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: a) Jobsite, 300 Halket Street, Pittsburgh, PA: On or about June 24, 2013, during steel erection activities, the employer did not ensure that fall protection systems/devices were in use where employees were exposed to fall hazards of up to approximately 16 feet, 6 inches.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Jobsite, 300 Halket Street, Pittsburgh, PA: On or about June 24, 2013, during steel erection activities, the employer did not ensure that the ladder, used to gain access to/egress from the steel deck, was extended at least 3 feet above the landing surface. The ladder was extended approximately 18" above the landing surface, exposing workers to fall hazards of up to approximately 16 feet, 6 inches.
