29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) Jobsite, 10090 Old Perry Highway, Wexford, PA 15205: On or before November 1, 2016, an employee that had been previously trained was unprotected and exposed to fall hazards of approximately 12 feet while operating an aerial lift.
