29 CFR 1926.351(b)(4): The employer did not ensure that cables in need of repair were not be used or that when a cable, other than the cable lead referred to in paragraph (b)(2) of this section, became worn to the extent of exposing bare conductors, the employer did not protect the portion exposed by means of rubber and friction tape or other equivalent insulation: a) Job site, 7225 McKnight Road, Pittsburgh, PA: On or about November 5, 2018, during welding operations, the employer did not ensure that weld cable leads were safe to use in that many areas of the jacket were damaged down to the interior wiring and were covered with electrical tape, and some areas had exposed bare copper wiring showing through with no covering of any type on them.
