29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.    On 7/8/2013, at the Braselton location, the employer did not ensure authorized employees were included in the periodic inspection performed for the Lockout/Tagout Program at the facility.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On 7/8/2013, in the Bar Support area, the employer did not ensure mechanical power presses were guarded as prescribed in Table )-10. Instances included:    1 - B6-1900 Federal Punch Press #3 (Opening was 8" and reach in distance = 6")  2 - B7 - 1914 Rouselle Press (Opening was 6" and reach in distance = 12")    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.255(b)(5): The hazard of flying sparks was not, wherever practical, eliminated by installing a shield guard of safety glass or suitable fire resistant plastic at the point of operation.  On or about 7/8/2013, in the Bar Support area, employees were exposed to flying sparks at the B-line machines as welding operations were not guarded.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).    On 7/8/2013, at the Braselton Georgia facility, employees were exposed to point of operation hazards on machines not protected with guarding. Instances include:    1. A10-1822 - Progressive Resistance Welder (Opening 3" at point of operation)  2. A11-1810 - Taylor Winfield Press Welder (Opening 2 3/8" at point of operation)  3. A18-6375 - Manual Snap Tie Machine (Opening between dyes is 3/4" to 7/8" at the crimping station, Operator located at point of operation with direct access to area)  4. E20-1565 - Automatic Snap Tic Machine (Opening between dyes is 3/4" to 7/8" at the crimping station, Operator located at point of operation with direct access to area)  5. B8-9556 - Eurobend Machine (Opening between light curtain and physical guard = 1' wide opening, reach in distance = 18")  6. B9-8037 - Eurobend Upper (Opening between light curtain and physical guard = 1' wide opening, reach in distance = 18")    Dayton Superior Corporations was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 314207614, citation number 1, item number 3 and was affirmed as a final order on 3/29/2011, with respect to a workplace located at 1372 Jesse Cronic Rd, Braselton Ga.

29 CFR 1910.217(e)(1)(ii): Records of inspections and the maintenance work performed on the clutch/brake mechanism, anti repeat feature, and single stroke mechanism of mechanical power press(es) were not maintained.  On or about 7/8/2013, in the Hold Down area, the employer did not ensure all mechanical power presses were inspected through a written certification record. Mechanical power presses that did not have a written certificaiton record included:  1 - D16-1560 Side Frame Johnson Press 2 - D17-1649 Havir Press Rite    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
