29 CFR 1910.217(e)(1)(i): Periodic and regular inspections of each power press did to ensure that all of its parts, auxillary equipment, and safeguards, including clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment: (a) Production - The employer did not include the inspection of the clutch/brake mechanisms during their monthly preventative maintenance inspections. On October 16, 2015, an employee suffered an amputation injury while operating a Minster 60 Ton part revolution press. The brake anchor pin set screws used to secure the clutch plate to the braking system were pulled away causing the clutch to rotate freely on the drive shaft of the press. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
