29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source:  Employees engaged in production activities at the following areas throughout the plant were exposed to fire and egress hazards in case of emergency and power loss in that the following emergency exit signs were not self-illuminating,  electroluminescent, or lit by emergency lighting especially during second shift with no sun light. a) South Building - The emergency exit sign without lighting (illumined), no back up power system, or battery operated, not glow in the dark type of exit sign. b) Main Plant by the WDA dock - The emergency exit sign without lighting (illumined), no back up power system, or battery operated, not glow in the dark type of exit sign.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in machining activities at the following pieces of equipment by the machine shop area were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized: a) Bridgeport Milling Machine Serial #J-56176, employees were not provided protection from flying chips and debris. b) Bridgeport Milling Machine Serial #J-97276, employees were not provided protection from flying chips and debris. c) Wilton #280 Milling Machine Serial #822082, employees were not provided protection from flying chips and debris. d) Jet Drill Press #DSC8250, the rotating drill chuck was not guarded. e) Bridgeport Milling Machine #DSC6296, employees were not provided protection from flying chips and debris. f) Bridgeport Milling Machine #DSC2120, employees were not provided protection from flying chips and debris. g) Bridgeport Milling Machine #DSC3203, employees were not provided protection from flying chips and debris. h) Sharp Milling Machine #DSC8292, employees were not provided protection from flying chips and debris. i) Walker Turner Drill Press Machine #DSC4468, the rotating drill chuck was not guarded. j) Johansson Drill Press Machine #DSC4482, the rotating drill chuck was not guarded.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Employees engaged in machining activities at the at the bench grinder (Dayton Commercial Bench Grinder Machine Model #4Z672B, 1/3 HP, 115 Volts, 3450 RPM) located in the machine shop area were exposed to struck-by hazards in that the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1 inch.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Employees engaged in production activities at the following areas throughout the plant were exposed to electrical shock and fire hazards from energized electrical wires. a) South Building - The service disconnect panel box by the air compressor has a knockout missing on the left side, and another knockout missing at the bottom of this electrical panel. Two knockouts missing for this panel box. b) Hand Run/Snap-Tie area - The service disconnect panel box between the Bliss Press (40-Ton C- Frame Press) and the LO/TO Center has a knockout missing at the bottom of this electrical panel.
