(a) Facility Wide - An evaluation of each powered industrial truck operator's performance was not conducted in the past three years. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(p)(1): Powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck was not taken out of service until it was restored to safe operating condition. (a) On or about July 11, 2013, the power industrial truck, Nissan 25, was not taken out of service until the horn was restored to safe operating condition. No abatement certifcation or documentation is required for this item.
