29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:   Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, housekeeping in the storage and workshop area was not maintained.  Clutter, equipment and materials were not kept in an orderly condition exposing workers to fire and other emergencies requiring safe egress from the facility.  Employees were also exposed to slips, trips and falls.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":   Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, the exit door throughout the facility were not clearly marked with an Exit sign.  Employees were exposed to injuries including smoke inhalation and burns in the event of a fire and other emergencies requiring safe egress.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, portable fire extinguishers in the facility were not properly mounted and not readily accessible. A lack of readily accessible extinguishers in emergencies that require a fire to be controlled or extinguished exposed workers to serious fire related injuries including smoke inhalation and burns.

29 CFR 1910.303(f)(2):  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto,   branch circuits on the facility's main breaker panel was not marked to indicate its purpose or location. Employees were exposed to electrical and mechanical hazards during emergencies and other situations requiring the shut off of the correct circuits and/or equipment.  Inaccurate labeling of circuit breakers in the electrical cabinet hinder and delay an employees' ability to identify and shut down the appropriate circuit breakers during an electrical or equipment emergency.  Delays in shutting down electrical equipment increase the potential for serious injuries to employees during an emergency.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment:  Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto,  the work space around the work center compound's main electrical switches was not readily accessible to employees due to the storage of materials in front and around the equipment.  This condition hinders and delays an employees' ability to access and shut down the appropriate electrical circuits during an electrical or equipment emergency.  Delays in shutting down electrical equipment increase the potential for serious injuries to employees during an emergency.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective.  Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, the electrical wall outlet near the entrance to the main work center compound's building had a faulty ground. Employees were exposed to accidental electrical shock hazards when connecting equipment, cords and extension strips.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a)    Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, the main electrical power panel had an open breaker exposing the energized conductors.   Employees were exposed to the hazard of electric shock while performing maintenance and other electrical work on and around the electrical panel.
