29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: Six Rivers National Forest, Hot Shots Smith River Compound and Work Center: On July 1, 2013, and at times prior thereto, housekeeping was not adequately maintained at the 10 Engine Bay and warehouse building. Clutter, equipment and materials were not maintained in an orderly condition, exposing workers to fire hazards and other emergencies requiring safe egress from the facility. Employees were also exposed to and slips, trips and falls.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit": Six Rivers National Forest, Hot Shots Smith River Compound and Work Center: On July 1, 2013, and at times prior thereto, doors designated as exits on the facility's evacuation map were not clearly marked with an Exit sign. Employees were exposed to serious injuries associated with limited safe egress from the facility during fires and other types of emergencies.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: Six Rivers National Forest, Hot Shots Smith River Compound and Work Center: On July 1, 2013, and at times prior thereto, branch circuits at the work center compound's electrical breaker panel were not properly labeled or marked to indicate its correct purpose or location. Inaccurate and missing labels on circuit breakers in the electrical cabinet hinder and delay an employees' ability to identify and shut down the appropriate circuit breakers during an electrical or equipment emergency. Delays in shutting down electrical equipment increase the potential for serious injuries to employees during an emergency.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective. Six Rivers National Forest, Hot Shots Smith River Compound and Work Center: On July 1, 2013, and at times prior thereto, three electrical wall outlets in the main administrative office had a faulty grounds. Employees were exposed to accidental electrical injuries when using the wall outlets to power equipment.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace: Six Rivers National Forest, Hot Shots Smith River Compound and Work Center: On July 1, 2013, and at times prior thereto, Material Safety Data Sheets were not available for Commercial Grade QuickCrete concrete containing Portland Cement. Employees did not have access to safety information on the exposures, hazards, and adequate protection when working with this material. They were exposed to first and third-degree burns and skin ulcers from the alkaline nature of the cement.
