29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:   Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, housekeeping in the storage and work shop was not properly maintained.  Clutter, equipment and materials were not kept in an orderly condition exposing workers to fire and other emergencies and slips, trips and falls.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.    Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto,  the work center compound's warehouse/workshop exit aisle was obstructed, limiting the width of the exit route to 26 inches.  Employees were exposed to serious injuries associated with limited safe egress of the building during fire and other emergencies.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":   Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, exit doors throughout the facility were not adequately marked with an exit sign.  Employees were exposed to serious injuries associated with delayed safe egress during fires and other emergencies including smoke inhalation and burns.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, portable fire extinguishers in the facility were blocked, not properly mounted, and not readily accessible exposing workers to serious fire injuries including smoke inhalation and burns in the event of an emergency.

29 CFR 1910.303(f)(2):  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, each branch circuit on the facility's main breaker panel was not marked to indicate its purpose or location. Employees were exposed to electrical and mechanical hazards during emergencies and other situations requiring the shut off of the correct circuits and/or equipment.    Inaccurate labeling of circuit breakers in the electrical cabinet hinder and delay an employees' ability to identify and shut down the appropriate circuit breakers during an electrical or equipment emergency.  Delays in shutting down electrical equipment increase the potential for serious injuries to employees during an emergency.

Six Rivers National Forest, Hot Shots Mad River Compound Work Center: On July 28, 2013, and at times prior thereto, the electrical wall outlet near the entrance to the main work center compound's building had a faulty ground. Employees were exposed to accidental electrical shock hazards.
