Employees using hand and power tools and exposed to the hazard of falling, flying, abrasive, and splashing objects, or exposed to harmful dusts, fumes, mists, vapors, or gases were not provided with the particular personal protective equipment necessary to protect them from the hazard. a) Jason Westhoven and Associates, LTD on or about July 17, 2013 at the address of 1401 Wren St. Bowling Green, OH: The employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to fasten asphalt shingles. Employees were exposed to potential flying object hazards while operating the pneumatic gun. Jason Westhoven and Associates, LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.300(c), which was contained in OSHA inspection number 98313, citation number 01, item number 01 and was affirmed as a final order on December 06, 2011, with respect to a workplace located at 248 Kolbe St. Napoleon, OH.

Employees were exposed to an 18 ft. fall to the ground below. Jason Westhoven and Associates, LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 98313, citation number 02, item number 02 and was affirmed as a final order on December 06, 2011, with respect to a workplace located at 248 Kolbe St. Napoleon, OH.
