A. Jason Westhoven and Associates, LTD worksite located at 3033 Algonquin Parkway, Toledo, Ohio: On or about May 16, 2017, the employer did not ensure the side rails of the extension ladder was extended at least three feet above the landing surface. Employees were exposed to falls while using the ladder to access the roof of the two story residential structure.

29 CFR 1926.300(c): Personal protective equipment. Employees using hand and power tools and exposed to the hazard of falling, flying, abrasive, and splashing objects, or exposed to harmful dusts, fumes, mists, vapors, or gases were not provided with the particular personal protective equipment necessary to protect them from the hazard. A. Jason Westhoven and Associates LTD worksite located at 3033 Algonquin Parkway, Toledo, Ohio: On or about May 16, 2017, the employer did not ensure an employees were provided with proper eye protection while operating a pneumatic nailers when installing weatherproofing on the two story residential structure. The employees were exposed to potential flying object hazards while operating the nailers. Jason Westhoven and Associates LTD was previously cited for a violation of this Occupational Safety and Health Standard 1926.300(C) which was contained in OSHA inspection number 1077715, citation number 1, item number 1 and was affirmed as a final order on August 20, 2015, with respect to a workplace located at Defiance, Ohio.

The employees were exposed to a potential fall distance of approximately 18.00 feet. Jason Westhoven and Associates LTD was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 920425, citation number 1, item number 2 and was affirmed as a final order on August 19, 2013 with respect to a workplace located at Bowling Green, Ohio.
