29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) Employees were exposed to head injury while working without protective helmets in an area below debris removal operations where debris was being thrown from the roof deck. Star Roofing & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100 (a) which was contained in OSHA inspection number 314173865, citation 1, item 2, and was affirmed as a final order on April 13, 2010, with respect to a worksite located at 418 E. 5th Street, Wilmette, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Employees were exposed to the hazard of falling approximately 1 story while disposing of roofing materials and debris at the roof edge where fall protection had not been provided. Star Roofing & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(1) which was contained in OSHA inspection number 190631, citation 1, item 1, and was affirmed as a final order on March 29, 2012, with respect to a worksite located at N/E corner of S West Avenue & St. Charles Road, Building 511-513, Elmhurst, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
