29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: a) On February 14, 2024, employees were exposed to eye injury while operating a pneumatic nail gun without eye protection during roofing work. b) On February 14, 2024, employees were exposed to eye injury while operating a hand operated staple gun without eye protection during roofing work. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On February 14, 2024, employees performing roofing work were exposed to fall hazards of approximately 18.6 feet. Star Roofing and Siding, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1716259, citation number 1, item number 1 and was affirmed as a final order on February 03, 2024, with respect to a workplace located at 4350 W Ogden Ave., Chicago, IL 60623. Star Roofing and Siding, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1609698, citation number 1, item number 1 and was affirmed as a final order on December 20, 2022, with respect to a workplace located at 1519 N Hudson Ave., Chicago, IL 60610.
