On or about July 25, 2013, at the above addressed job-site, employees were exposed to a 17-foot high fall hazard when an unsecured fixed ladder was not withdrawn from service.

Boynton Beach Blvd., Boynton Beach, FL. On or about July 24, 2013, at the above addressed job-site, employees were exposed to a 17-foot high fall hazard when a ladder that was unsecured was not inspected by a competent person prior to be used again.
