29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about February 25, 2025, at the worksite located at 8970 W State Road 84, Davie, FL 33324, at the back of the building, an employee was exposed to ejection and fall hazards while working from the platform of a JLG articulating boom lift (Model #450 AJ, Serial # 0300285072) at a height of 13.7 foot without being protected from falling. b) On or about February 25, 2025, at the worksite located at 8970 W State Road 84, Davie, FL 33324, at the front of the building, an employee was exposed to ejection and fall hazards while working from the platform of a JLG Telescopic boom lift (Model #400S, Serial # 0300225110) at a height of 13 foot without being protected from falling.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about February 25, 2025, at the worksite located at 8970 W State Road 84, Davie, FL 33324, on the roof at the back of the building, an employee was exposed to a 13.7-foot fall hazard while installing a metal roof. b) On or about February 25, 2025, at the worksite located at 8970 W State Road 84, Davie, FL 33324, on the roof at the front of the building, an employee was exposed to a 13-foot fall hazard while installing a metal roof.
