29 CFR 1910.23(a)(6): Manhole floor opening(s) with the cover(s) removed were not constantly attended by someone or protected on all exposed sides by removable standard railings.     a.     On or about 9 Aug 2013, at the Water Treatment Plan, USDA-El Yunque, Puerto Rico.  Unaccompanied employees engaged in water treatment operations had been accessing the manholes entrance at the top of the Sedimentation Tank with missing standard railings.  The maximum fall from the top of the manholes entrance to the ground level was 9 feet high.       NOTE:   Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a.     On or about 9 Aug 2013, at the Water Treatment Plan, USDA-El Yunque, Puerto Rico.  Employees engaged in water treatment operations had been accessing a platform with missing railing on all sides where the entrance to the Sedimentation Tank is located.  The maximum fall from top of the platform was 9' to the ground level.         NOTE:   Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    a.     On or about 9 Aug 2013, at the Palo Colorado and Caimitillo Deep Well Pump Stations, USDA-FS, El Yunque, Puerto Rico.  Employees engaged in water treatment operations have been connecting electrical tools and equipment such as water pumps to metal outlet boxes located inside of each of the two pump stations. Each metal outlet box have missing a face plate cover.      NOTE:   Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
