29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about August 14, 2013, at the east side of the house, an employee using an air nail gun to install drip edge metal was not wearing eye protection, exposing himself to being struck by flying debris or nails. Great White Construction , Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 447093, citation 01, item 01, issued on June 4, 2012, with respect to a workplace at 12957 Everett St, Jacksonville, FL, 32225. The final order date was June 27, 2012.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about August 14, 2013, at the east side of building, one employee installing drip edge metal on a 4:12 pitch residential roof were not protected from an 8-foot fall hazard by the use of conventional fall protection system. b. On or about August 14, 2013, at the southwest side of building, two employees installing weather resistant underlying material on a 4:12 pitch residential roof were not protected from an 8-foot fall hazard by the use of conventional fall protection system. Great White Construction , Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 447093, citation 01, item 02, issued on June 4, 2012, with respect to a workplace at 12957 Everett St, Jacksonville, FL. The final order date was June 27, 2012.
