29 CFR 1926.451(d)(1): All suspension scaffold support devices, such as outrigger beams, cornice hooks, parapet clamps, and similar devices, did not rest on surfaces capable of supporting at least 4 times the load imposed on them by the scaffold operating. a) Jobsite: 11th floor west parapet wall - On or about 8/8/13, the surfaces used to support the cornice hooks were not capable of supporting 4 times the load imposed on them by the rated load of the hoist, or 1.5 times the stall capacity of the hoist. The employees were conducting masonry restoration operations on the west side of the building.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person. a) Jobsite: 11th floor west parapet wall - On or about 8/8/13, no competent person identified existing and predictable hazards during the installation of the horizontal lifeline used to re-position the suspension scaffold, and no prompt corrective measures were taken to eliminate them. The horizontal lifeline which was supported by visibly cracked balusters, and used without their capacity rating identified, put employees re-positioning the suspensions scaffold from the 11th story of the building at risk of falling.
