Observed on or about 8/24/15.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. a) Jobsite: 11th floor, south side - No competent person supervised or directed the erection of the suspension scaffold in a manner that ensured the safety of the employees working from the scaffold. The outrigger beams were not restrained from movement and the lifelines and the support lines were connected to the same anchor point, exposing employees to fall hazards. Observed on or about 8/24/15.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms. a) Jobsite: 11th floor, south side - The anchors used by employees to attach their personal fall arrest system were not independent of the anchors used to support the suspension scaffold. Both the personal fall arrest system lifelines and the suspended scaffold support lines were connected to two eye bolts on a metal plate that was secured to the face of the building. This condition exposed employees to fall hazards. Observed on or about 8/24/15.
