A hoisting device equipped with a brake capable of stopping uncontrolled movements was not utilized to prevent the scaffold from dropping.  This was documented to occur on August 8, 2018.

The manually operated suspension hoists were not equipped with positive crank force devices to control descend and prevent the scaffold from dropping.  This was documented to occur on August 8, 2018.

29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:  Jobsite:  3525 I Street, Philadelphia, PA 19134 building Rooftop - Employees conducting masonry work from a two point suspended scaffold on a 5th story window were exposed to fall hazards.  Each employee was not adequately protected from falling.  a)  An employee wearing a full body harness was provided a vertical synthetic rope lifeline to utilize with a shock absorbing lanyard and rope grab, that did not prevent the employee from falling.  This was documented to occur on August 8, 2018 when the employee fell approximately 30' from the scaffold.    b)  An appropriate guardrail system consisting of a top railing measuring between 36" and 45" above the platform and either midrails, screens, mesh, or other intermediate vertical member no more than 19" apart were not provided on the scaffold.  This was documented to occur on August 8, 2018 when the employee fell approximately 30' from the scaffold.

29 CFR 1926.451(g)(3)(i): Verical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion.  Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights.  a) Jobsite:  3525 I Street, Philadelphia, PA 19134 building Rooftop - Employees conducting masonry work from a two point suspended scaffold on a 5th story window were exposed to fall hazards.  A vertical life line used by an employee was tied around an HVAC vent instead of a safe point of anchorage.  Vertical life lines were also not protected from sharp edges where they were draped over the building's parapet wall.  This was documented to occur on August 8, 2018.

Sound points of anchorage include structural members, but do not include standpipes, vents, other piping systems, or electrical conduit.  a) Jobsite:  3525 I Street, Philadelphia, PA 19134 building Rooftop - Employees conducting masonry work from a two point suspended scaffold on a 5th story window were exposed to fall hazards.  A single tie back used to secure one of the two parapet hooks was not installed at a right angle to the building's parapet, nor was a structurally sound point of anchorage used to secure the tieback as it was attached to an HVAC unit.  This was documented to occur on August 8, 2018.  Melrose Enterprises, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(d)(5)(iii), which was contained in OSHA inspection number 931310, citation number 1 item number 3 and was affirmed as a final order on February 14, 2014, with respect to a workplace located at 170 S Independence Mall W, Ste 960, Philadelphia, PA 19106.
