29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about 08/16/2013 at 2624 Weaver Way Atlanta, GA; employees working at the M6, high volume line were exposed to electrical shock and burn hazards where:  a. the 120V metallic quad-receptacle at the in-feed conveyor to case packager was used as a pendant powering the Dayton model 1UM36 floor fan, the take-away conveyor from APM, in-feed conveyor, and the 120 V plastic dual receptacle (Reverse wired)  b. the 120V plastic duplex receptacle that was powered by reverse wiring and by daisy chain from the metallic quad receptacle was used to power the in-feed conveyor and Dayton hard mounted fan.  c. a 120 V plastic duplex electrical box was used to power by daisy chain a 120 V metallic duplex box that was used to power a pump and to power via daisy chain a 120 V metallic duplex box that was used to power a 90 V Direct Current motor and a glue gun.  American Air Filter Corporation, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(2), which was contained in OSHA inspection number 312524432, citation number 01, item number 008a and was affirmed as a final order on 04/10/2009, with respect to a workplace located at 2624 Weaver Way Atlanta, GA.

A 120 V power switch for the fan was not closed at the electrical line of the M6 line;  c. A 120 V power box cover at the M6 line was not effectively closed;   American Air Filter Corporation, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(1)(ii), which was contained in OSHA inspection number 312524432, citation number 01, item number 008c and was affirmed as a final order on 04/10/2009, with respect to a workplace located at 2624 Weaver Way Atlanta, GA.
