29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about February 23, 2015 and times prior to, employees were exposed to struck-by hazards when equipment was not locked out during the following:  a.     Employees adjusted a sensor behind a guard at the rotary table without utilizing the LOTO procedure for the rotary table.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance:    On or about March 13, 2015, each employee involved in service and maintenance activities did not apply his/her own lock when performing activities such as, but not limited to, clean up in the Perfect Pleat Department. Employees were exposed to struck-by hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    A) On or about March 13, 2015,  employees were exposed to struck-by hazards when equipment in the Perfect Pleat Department was not locked out during the following:      Maintenance technician was installing a guard on Perfect Pleat Line #5 without locking out the airline. He only locked out the electricity.    B)   On or about March 13, 2015, employees were exposed to struck-by hazards when equipment in the Perfect Pleat Department was not locked out during the following:          During cleanup the airline was not locked out. Only the electricity was locked out on lines #5.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about March 5, 2015, employees were exposed to struck-by, caught-in, and amputation hazards when working on or walking near the Perfect Pleat lines #5, #10, and #3.      a. The guard on the tractor tribar on line #5 is not secured and is opened during setup in order to monitor alignment.   b. The guard on the tractor tribar on line #3 is not secured and is opened during setup in order to monitor alignment.   c. The saw is inadequately guarded on line #5.   d. The saw is inadequately guarded on line #3.   e. The saw is inadequately guarded on line #10 and the guard was missing a bolt.    f.  Pleat pack inserter is inadequately guarded on line #5.  g. Pleat pack inserter is inadequately guarded on line #10.  h. Pinch points on underside of conveyor that transfers product from the saw cut to the rotary table on line #5 were inadequately guarded.  i. Roll form rollers that bend the frame are not guarded on line #5.  j. Roll form rollers that bend the frame are not guarded on line #10.  k. Back side of rotary table on line #5.  Rotary table exit is open and inadequately guarded.  l. Glue dispensing system next to the rotary table on line #5 has no guard on the backside.  m. Loose guard on line #3 die cut machine.  n. Line #3 glue dispenser guard has no top or bottom.  o. employees were exposed to hazards of in-running nip points and/or injury resulting from conveyor belt breakage on line #3 at the belt accessible from end and bottom of guard.    The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 931623, citation number 1, item number 2 and was affirmed as a final order on 02/19/2014, with respect to a workplace located at 2624 Weaver Way, Atlanta, GA.    The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 978590, citation number 2, item number 1 and was affirmed as a final order on 06/26/2014, with respect to a workplace located at 2624 Weaver Way, Atlanta, GA.
