29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible.  On or about 5/21/14, employees were exposed to struck by and caught between hazards at the M6 line at the glue station as the guard the employer installed was not effective as there were openings on the sides measuring 18 inches x 3 feet wide and the frontal guards were not secured and could be opened while the machine was running.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.219(e)(4): Vertical belts running over a lower pulley more than seven (7) feet above floor or platform were not guarded at the bottom in the same manner as horizontal overhead belts, if conditions are as stated in paragraphs (e)(2)(ii) (a) and (c) of this section.  On or about 5/21/14, employees were exposed to amputation hazards in the maintenance shop as the Millport machine belt and pulley was not guarded.    No abatement certification or documentation is required for this item.

On or about 5/21/14, employees were exposed to amputation hazards at the M6 Line as chains and sprocket drives were not guarded at various locations on the line and were within 7' of the floor.    No abatement certification or documentation is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, and flying chips.  On or about 5/21/14, at the 2624 Weaver Way, Atlanta, Georgia site, employees were exposed to struck by and caught between hazards at various locations in the production area. Instances included:  1 - M6 glue stations 2 - Conveyor bars on the M6 line 3 - at the M6 Strip Feeder  4 - at the Transfer Station incline belt conveyor - abated during inspection 5 - M6 Case Packer conveyor - abated during inspection 6 - Drill Press in Maintenance shop (Serial #AAF 104710) - abated during inspection 7 - Millport machine in Maintenance shop (No serial #) - abated during inspection   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.  American Air Filter Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 931623, citation number 1, item 2 and was affirmed as a final order on 2/19/2014, with respect to the workplace at 2624 Weaver Way, Atlanta, Ga.
