29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:    On or about October 20, 2014 and at times prior to, employees operated HV Lebanon machine with a missing guard, exposing employees to struck-by and caught between by the moving pusher/conveyor bars.  Employees performed tasks including   but limited to cleaning/removing glue operations, oil the rails from the machinery, such as but limited to the 2nd leg of HV Lebanon line.  Employees were about 6-24 inches from the pusher/conveyor bars.  The guard was removed in the middle of the 2nd leg of HV Lebanon line.  HV Lebanon machine produce air filters.     American Air Filter Company, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 978590, citation 2, item number 1 and was affirmed as final order June 26, 2014, with respect to workplace at 2624 Weaver Way, Atlanta, GA 30340.  (HV Lebanon machine is similar to HV M6 and HV Dallas machines).
