29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a ridgid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Worksite: The single piece ladder that was used to gain access to the second floor window of the residence under construction at a height of approximately 14 feet above the ground did not have its side rails extended 3 feet above the landing surface and a grasping device was not provided.

Worksite: The employee who was framing the residence under construction while exposed to fall hazards above six feet was not protected from fall hazards by the use of guardrails, safety nets or a personal fall arrest system.    SLP Contractors, LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(14) which was contained in OSHA inspection number 311798615, citation number 1, item number 4, issued on 4/19/2010 with respect to a workplace located at 1092 Farmington Ave., West Hartford, CT and became a final order on 5/12/2010.
