29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program would enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:   A) On or about July 29, 2020, at the 100 Lexington Street, Windsor, CT job site:  Employees were observed working unsafely on ladders and roof surfaces.  The employer did not provide a training program for each employee who was potentially exposed to a fall hazard.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   A) On or about July 29, 2020 at the 100 Lexington Road, Windsor, Ct job site:   The employer did not provide a ladder at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more.  Employees jumped over the five foot break in elevation from lower level (patio working area) to the main walking and working surface.

29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note :  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.   A) On or about July 29,  2020 at the 100 Lexington Road, Windsor, CT job site:   Two employees were caulking around a window, working  from an aerial lift.   The two employees did not have a body belt with a  lanyard attached to the boom or the basket.  The aerial lift basket was approximately 20 feet from the ground below it. Employees were exposed to a fall hazard.  The SLP Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 1926.453(b)(2)(v),  which was contained in OSHA inspection number 1460304, citation number 1, item number 2, group A, and was affirmed as a final order on June 23, 2020, with respect to a workplace located at 100 Lexington Road, Windsor, CT.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.     A) On or about July 29, 2020 at the 100 Lexington Road, Windsor, CT job site:    Two employees were working on a deck platform area,  five feet above the lower level and 12 feet above the ground below; and there was no guardrail installed on the two open edges.  This deck work area was on the second floor of the structure. The employees were not provided fall protection.   The SLP Contractors, LLC,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13),  which was contained in OSHA inspection number 1405520, citation number 1, item number 1 and was affirmed as a final order on September 16, 2019, with respect to a workplace located to 100 Lexington Road in Windsor, CT.
