29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 03/10/2020 at the Preserve at Great Pond Construction Project, employees were using pneumatic nails guns and circular saws and were not wearing or using any eye protection when exposed to eye inpact hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  On or about 03/10/2020 at the Preserve at Great Pond Construction Project, the foreman was working from an aerial lift and was not using personal fall protection.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about 03/10/2020 at the Preserve at Great Pond Construction Project, the foreman and an employee were working from aerial lifts and were using personal fall arrest systems that allowed contact with the lower levels.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 03/10/2020 at the Preserve at Great Pond Construction Project, the foreman was working from the 2nd and 3rd story decks and was not using any fall protection system.  SLP Contactors LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1405520, citation number 01, item number 001 and was affirmed as a final order on 09/16/19, with respect to a workplace located at 100 Lexington Road in Windsor, CT.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about 03/10/2020 at the Preserve at Great Pond Construction Project, employees were using two extension cords to energize their power tools and the extension cords had strain relief damage.
