The operator had four 42" pallets stacked on the forks, obstructing his forward view.  The operator was not directed to travel with the load trailing.

On or about August 28, 2013, Warehouse:  A powered industrial truck operator did not look in the direction of travel.  The operator had pulled into an aisle and backed up without looking behind the truck.  The operator was facing forward, directing another operator where to place a load.  This condition resulted in an employee being struck by a powered industrial vehicle, sustaining serious injury.
