29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that would not deflect, nor was a grasping device provided to assist employees in mounting and dismounting the ladder:    a. On or about September 11, 2013, to access the 6:12 pitch roof, an employee used a 10-foot portable metal folding ladder that could only extended 16 inches above the landing surface.  The ladder was not secured, nor was a grasping device provided, exposing the employee to an 8-foot 9-inch fall hazard.    The Bobby Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 313877292, citation number 01, item number 02 and was affirmed as a final order on August 13, 2010, with respect to a workplace located at 1915 Dumfries Court, Jacksonville, FL 32259.
