29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.   Employees installing framing and sheathing at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system:    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on December 18, 2015, when employees installing a shingle roofing system on the 6:12 pitch roof of a house at 558 Pullman Circle in Saint Augustine Florida were exposed to 8 and 10 feet fall hazards above lower levels in that they did not have properly fastened roof anchor points to attach personal fall arrest gear, and they did not have their personal fall arrest gear fastened.    Bobby Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1042466, Repeat citation number 1, item number 1 and was affirmed as a final order on May 26, 2015, with respect to a workplace located at 360 Sanctuary Drive, Saint Johns, Florida.    Bobby Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 998292, Repeat citation number 2, item number 1 and was affirmed as a final order on February 27. 2015, with respect to a workplace located at 6365 Rolling Tree Street, Jacksonville, Florida.    Bobby Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 961128, Repeat citation number 2, item number 1 and was affirmed as a final order on May 27, 2014, with respect to a workplace located at 738 West Kings College Drive, Saint Johns, Florida.    Bobby Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 951873, Repeat citation number 1, item number 1 and was affirmed as a final order on February 27, 2014, with respect to a workplace located at 2344 Open Breeze Court, Green Cove Springs, Florida.    Bobby Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 940850, Repeat citation number 1, item number 1 and was affirmed as a final order on October 31, 2013, with respect to a workplace located at 244 West Adelaide Drive, Saint Johns, Florida.
