29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Near Building 26196 - Fuel Management On or about June 5, 2013, and at times prior thereto, the outer insulation used on the Wacker concrete mixer was pulled away from the plug creating a hazard to the employees using the mixer during pouring concrete.
