29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system: a) West 2nd & Jefferson St, Philadelphia, PA - The employees were working approximately 30 feet above the ground without the use of any personal fall arrest system, thus exposing the employees to a fall hazard greater than 6 feet.This violation was observed on or about September 5, 2013. The employer, Five Star Quality Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 699658, citation number 1, item number 1 and was affirmed as a final order on , with respect to a workplace located at West 2nd & Jefferson St, Philadelphia, PA. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
