29 CFR 1910.119(e)(3)(iv): The process hazard analysis did not address the consequences of failure of engineering and administrative controls:         (a)  BP-Husky Refining LLC, Oregon, Ohio:  On or about September 24, 2013, the process hazard analysis of the lean amine filter (PR510201) did not address the consequences, as they relate to employee safety, when the particulate filter was bypassed due to plugging of the filter media.  Employees were at risk of illness from exposure to hydrogen sulfide and sulfur dioxide when the thermal oxidizer failed because of particulates in the process stream.    AVD Amended to read:  BP-Husky Refining LLC, Oregon, Ohio:  		On or about September 24, 2013, the process hazard analysis PR510201 addressed the external by-passing of the lean amine filters impact on employee safety, however, did not evaluate the consequences of internal by-passing of the filter.

The procedure should include, but not be limited to, change out schedule and frequency, guidelines for manual bypassing of the filter, and operational responses to high differential pressure across the filter.  Failure of the filter exposed employees to hydrogen sulfide and sulfur dioxide hazards who were at risk of serious illness.    AVD amended to read:  BP-Husky Refining LLC, Oregon, Ohio: On or about September 24, 2013, the employer did not assure that written procedures for normal operation associated with the lean  amine filter included specific guidance for changing the filter based on amine quality and high differential pressure across the filter.

Bypassing the filter resulted in particulates reaching the absorber (PR550017) and plugging the absorber trays.  Employees were exposed to hydrogen sulfide and sulfur dioxide hazards and at risk of serious illness when liquid from the absorber caused the thermal oxidizer (PR560005) to fail.
