(b) Production - The employer did not ensure that a pit over 6 feet deep located at Slitter 581 near the coil loading zone was guarded with a standard railing, which consisted of a top rail, intermediate rail and posts, and had a vertical height of 42 inches nominal from upper surface of top rail to floor.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by nip points and rotating parts:      (a) Press 311/Press 438/Press 437- The employer failed to ensure that all permanently fenced doors surrounding the mechanical power presses and movable fence doors equipped with wheels located in-front of the mechanical power press were interlocked to prevent access to areas around the press where rotating parts were present. The interlocks on some doors were broken, missing or never installed in several areas of the presses.        (b) Slitter 581- The employer failed to ensure that all permanently fenced doors were equipped with working interlocks, some of the sensors were bent or broken off and some of the fenced doors were not equipped with interlocks at all. Stairs leading to the platform where slitter knives are present were not guarded to prevent employees from accessing the area.      (c) USA 332 - The employer failed to ensure that permanently fenced doors extended to all areas that would allow employee access where ingoing nip points and rotating parts were present. Some interlocks equipped on permanently fenced doors were bent, broken off and some fenced doors were not equipped with interlocks but latches. The fencing equipped did not fully extended to prevent employees from being exposed to rotating and moving parts.       (d) USA 149 - The employer failed to ensure that permanently fenced doors were present at all access areas that would allow employees entrance where ingoing nip points were present. Some interlocks equipped on permanently fenced doors were bent, broken off and some doors were not equipped with interlocks at all. Guard fencing did not extended to the area from which the material was ejected. Stairs allowing employee access to the back end of the machine where rotating parts were present was not guarded. A gated door near the coil encasing area was not interlocked allowing employees access into the back of the machine where rotating parts were present.         (e) Rollformers - The employer failed to ensure that permanently fenced doors were equipped with working interlocks, to prevent employees from accessing areas where moving parts were present. Some of the interlocks on the machine were broken off or missing.
