29 CFR 1910.94(a)(4)(i): The construction, installation, inspection, and maintenance of exhaust systems shall conform to the principles and requirements set forth in American National Standard Fundamentals Governing the Design and Operation of Local Exhaust Systems, Z9.2-1960, and ANSI Z33.1-1961, which are incorporated by reference as specified in �� 1910.6. a) On or about October 1, 2013 inspection and/or maintenance was not performed on the Tinker Omega Hopper unit to determine blockage and/or slowdown within the suction unit, resulting in the over head duct work collasping due to the weight of the sand.

29 CFR 1910.94(a)(4)(i)(b): The static pressure drop at the exhaust ducts leading from the equipment shall be checked when the installation is completed and periodically thereafter to assure continued satisfactory operation. Whenever an appreciable change in the pressure drop indicates a partial blockage, the system shall be cleaned and returned to normal operating condition. a) On or about October 1, 2013 static pressure inspection checks on the Tinker Omega hopper were not performed periodically to determine blockage within the hopper, resulting in the collaspe of the over head ducting system. b) On or about October 1, 2013 inspection and cleaning of the duct work system on the Tinker Omega hopper were not performed to ensure proper airflow resulting in the collaspe of the over head ducting system.
