29 CFR 1910.184(e)(3)(i): In addition to the inspection required by paragraph (d) of this section, a thorough periodic inspection of alloy steel chain slings in use shall be made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Such inspections shall in no event be at intervals greater than once every 12 months.    The employer had not conducted an inspection of alloy steel chain slings used in the facility at an interval no greater than once every 12 months.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    In the Air Set Department, an employee was exposed to an electric shock hazard from the missing cover to the junction box on the Palmer Sand Mixer machine.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from struck-by hazards from the use of below-the-hook lifting devices.    In the Production Area, employees were exposed to a struck-by hazard by untested, unrated and unmarked below-the-hook lifting devices in conjunction with the 10 and 15 ton overhead bridge cranes when lifting furnace pots that weight approximately 25 to thousands of pounds with a furnace pot lifting hooking beam.      Among other methods, one feasible and acceptable method to correct this hazard is to follow the guidelines in American Society of Mechanical Engineers (ASME) B30.20-2006, Below-the-Hook Lifting Devices, Section 20-1.2.1, wherein it specifies that below-the-hook lifting devices must be capable of lifting loads within it rated capacity, which must be marked so as to inform the user to prevent overloading and failure of the device.  The employer shall conduct frequent and periodic inspection of these devices to ensure that the rated capacity is marked and used within it rated load capacity.    A & B Foundry & Machining LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 736661, citation number 1, item number 1 and was affirmed as a final order on December 18, 2013, with respect to a workplace located at 835 North Main Street, Franklin, OH.

On or about April 5, 2016, in the Pattern Shop, an employee was exposed to an amputation hazard while using an unguarded saw blade on the Delta table saw.
