29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used.    (a) At the Naval Base Guam Fuel Farm, behind buildings 1704 & 1704A, an employee utilized a Makita 9557NB 4-1/2-Inch Angle Grinder to cut No. 5 rebar.  The power cord to the grinder was worn and frayed, thus exposing employees to potential electrical shock and/or burns requiring medical treatment.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest.    (a) At Naval Base Guam Fuel Farm, between building 1704 & 1704A, employees applying roof coating on building 1704 were not protected from falling through a 11 ¾ gap between the roof top of building 1704 and 1704A.  Although a guardrail system was in place, the guardrail was erected on building 1704A.  Employees were exposed to a fall hazard of about 18 feet from the roof to the ground.

29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  Note:  When employees are using stilts, the top edge height of the top rail, or equivalent member, shall be increased an amount equal to the height of the stilts.    (a) At Naval Base Guam Fuel Farm, Building 1704, on October 30, 2013 and at times prior thereto, the top edge height of top rails were not positioned 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level.  The distance from the working platform to the top rail was approximately 56 inches.  Employees used the frame type scaffold to gain access to the roof edge, in order to repair spalling concrete.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    (a) At Naval Base Guam Fuel Farm, Building 1704, on October 30, 2013, no ladder, stairs, or ramp, were provided to access the frame type scaffold.  The distance from the ground to the side access of the frame scaffold was 29 ¾ inches.  Employees climbed the end frames of the frame type scaffold to access the working platform.  Employees used the scaffold to access the roof eave to repair spalling concrete.
