29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions had been taken to ensure their safety: (a) On or about October 31, 2013, for employees exposed to cave-in hazards while installing a sewer line approximately 19 feet deep without protective systems where one employee was injured when a trench wall caved in at or near 600 Knutson Street, Mapleton, North Dakota.
